DOL Dumps LS-206 and Revises LS-208

The Longshore Division of the U.S. Department of Labor has phased out the old form LS-206 and incorporated elements of that form into a revised LS-208.  The old form LS-206, entitled "Payment of Compensation Without Award," will no longer be used.  Instead, the form LS-208, formerly known as the "Notice of Final Payment or Suspension of Compensation Payments," will serve as the primary form used to report all payments of compensation.  The new LS-208 will simply be known as the "Notice of Payments."  The Longshore Division's notice regarding this change may be viewed here.
